I am writing this letter in response to your (letter or telephone call) on (date) wherein you informed me that I owe $______ .
Since I do not believe I owe this debt, I am requesting that you comply with Section 809 of the Fair Debt Collection Practices Act and send me the following information:
(1) The name of the original creditor;
(2) What goods or services were purchased with the money you claim I owe;
(3) All documentation proving that I owe this debt; and
(4) A copy of your license to practice debt collection in my state. [delete (4) if addressed to an attorney]
I am also requesting that you send a copy of this letter to the original creditor notifying them that I dispute this debt so that they cannot report it as delinquent on my credit report. Likewise, if you have reported this debt to any credit bureau, I request that you also report to them that I dispute owing the debt. Please notify me when you have done so.

Dear Sir/ Madam:
I am writing this letter to dispute the collection account of (name of collection agency or attorney) on my credit report and am requesting it be deleted as soon as possible.
On (date), I notified (name of collection agency or attorney) that I did not owe this debt. I also demanded that they not report this account to any credit bureau and asked them to provide me with proof that I owe the debt. Attached is a copy of the certified letter I sent them along with copies of the certified mail receipt showing that they received it.
Not only did they fail to provide me with proof that I owe the debt, they reported this information to my credit report anyway, which is a blatant violation of Section 809 of the Fair Collection Practices Act.
I am demanding that you remove this account from my credit report at once and send me a corrected copy. Please remove this information and send me a corrected copy of my credit report as soon as possible.
